  • Dr Chappell does not accept any 3rd party payer.  Payment is fee for service at check in via cash, credit card, check or money order.  You can then submit your bill to your insurance or HSA for reimbursement.

What billing or insurance information will I receive?

​You will receive a copy of your super-bill at the end of the appointment.  You can then submit the bill to your own insurance for reimbursement provided that your deductible has been met.
